About Your Visit

Your Appointment and the Patient Portal

 New Patients (or patients not seen in the last 2 years) should plan to arrive 30 minutes prior to their appointment time to ensure adequate time to complete forms and confirm insurance.  


We encourage both new and established  patients to use our secure Patient Portal to  enter their information and prepare for their visit.  Our patients can enjoy the convenience of reviewing their lab results and requesting prescription refills through the portal.  


If you do not have access to your portal login, or are having difficulty on the website, please contact our office so we may help you login.  Our friendly front desk staff is happy to set up portal access for you, or walk you through set-up if you need assistance.  

Patients with no internet access or cannot otherwise access their portal are encouraged to download the New Patient Paperwork available on this website.  This paperwork must be delivered to our office in advance of your visit to ensure a smooth registration for you during your appointment.  

 
 

Some insurance plans require a prior authorization from your PCP prior to seeing a specialist. If you have an HMO, this may have to be obtained prior to scheduling your appointment.

Your Insurance card (s)


Your drivers license


A list of current medications and the pill bottles of current medicines 


Any co-payments required at the time of your visit


X-rays or diagnostic imaging other physicians have ordered and given to you.


If you have requested that medical records, test results or x-rays be forwarded to our office,  please confirm receipt of those records with our office to ensure that they will be available for our physician to review during your appointment.    

Office Hours and Emergency Contact Policy

Our office is open 8:00 to 5:00 Monday through Friday.  We are closed on weekends and holidays.  Refills and questions of a non-emergent nature can only be handled during regular office hours. 


When our office is closed,  our physicians are available for urology emergency calls only.  If you have an true urologic emergency, the urologist on emergency call for the practice can be contacted through the hospital page operator. 912-466-7000.

 At Southeast Georgia Urology we are committed to treating and using protected health information about you responsibly. This Privacy Policy  describes the personal information we collect, and how and when we use or disclose that information. It also describes your rights as they relate to your protected health information.   Copies of our privacy policy are on display at our office and can be downloaded from our website.
